
Supplement Refill Request
Supplements are prescribed by Dr. Kaler, ND, to assist your healing and health journey. They are not permanent recommendations.
Your treatment plan will indicate how long your supplements are available after your last visit. If you are unsure, you can refer to your treatment plan (Also available on your JaneApp portal). Certain conditions or herbs have a maximum duration during which they can be used safely.
After this time period, you are required to book a follow-up visit for reassessment and to ensure the supplements are still appropriate and safe.
Refill requests must be completed using the form below. Refills will not be accepted over the phone, via email or on a walk-in basis. For your privacy, only your Naturopathic Doctor has access to this information and our patient coordinators are not able to determine what is appropriate. Once your request is received, your Naturopathic Doctor will confirm and approve it before it is available for pickup.
Please complete the form below completely. Incomplete forms (ie, missing name, brand, etc.) can lead to delays, and you may be required to complete the form again if we cannot determine what you are looking for.
If you are not sure of the product you are looking for, the information is on your invoice and treatment plan. If you are not sure or cannot find this information, then book a follow-up visit to discuss.
